unable to complete gear up quest, when right clicking the charm if the sniper, it says "no availble item to apply charm"

Portal back into completed personal vault resulted in spawning in mostly sky with some vault looking surrroundings but unable to enter. Using stuck or respawn point didn't resolve. Quit game and tried again with same result.

Attempted to do the 5 piece agility puzzle but one of the totems was stuck in another building. No way to access or activate through wall.

Looks like underwear cliping through shoes, shirt clipping through jacket.

In the forest biome there is natural foliage that clips through built foundations. I imagine this'd be the same across other biomes. This foilage is non-destructible or collectible
