I was able to point-blank an enemy in the face with a rifle, however damage was not being taken. I expended roughly 50 rounds in testing this, with headshots registering at seemingly random intervals without moving the reticule.
They take no damage, as with body shots, in the majority of shots. Seems to work fine on most animals, but not all.

Thank you for bringing this to our attention. To confirm, what creature type were the enemies that you were headshotting? Were they consistently the Bound?
Extra Update: I noticed that, when getting a Heartshot, the heartshot icon comes up and shows that I did one... but it doesn't deal damage. Hopefully this should reduce the sense of "Skill Issue"

No, it was both the bound and some scorpions. Additionally, there were some of the Jackal things but I don't know what they are called proper.
