When talking to Desma the dialogue option "I've more legs of this Empyrean Network to open." can be taken two times successively, as shown in the image. One of these should be removed, or else a flag added so that a different dialogue option is given if it was used the first time since it's actually part of a different branch of the conversation.

Update, as this is still an issue:
Talking to Desma after finding 3/4 of her notes gives the "Are you Desma, by chance?" conversation option. Talking to her again after getting the 4th note and administering the potion to Ortega (not sure which condition mattered) gives the same conversation option a second time, and the same conversation options as the first time, but this time with the options to say it was tested on the mushroom/carnute and receive a reward. Ortega had already been cured, but she still gave the quest to cure him again, and the next conversation option was to say that he had been cured.
-This conversation could have been skipped and the reward not given (probably) if continuing the quest as normal rather than taking the repeated conversation option "Are you Desma, by chance?"
-Conversation loops need to be checked for this and other conversations for similar issues.
