I was killed by a berserker, and then was killed. When I got revived by a follower, I could note move at all with WASD. I could dash but and look around but no moving. I have this issue happen when I I use the Q dial for food while moving but I can usually stop pressing W and press it again to fix it but it did not fix it in this situation.

To clarify, do you have any custom key-bind settings, or are you playing with the default binds?
