I've noticed that equipped tools and Q wheel items will unequip when dealing with inventories like chests. It took some prodding to figure out how to trigger it, but if you open an inventory like a chest and doubleclick a non-equipped tool to move that item from your inventory to the chest, your currently selected toolbar tool will become unequipped and go into your inventory, leaving that toolbar slot empty.
The same applies to Q wheel items. Move a food item out of your loose inventory into the chest's inventory via doubleclick and whatever is in the top slot of your Q wheel gets pulled out of the wheel and into your inventory and now there's nothing on the quick Q assignment.
As far as I can tell this was introduced with today's update.
I was also able to reproduce this with the consumables wheel. Since double-clicking in the Inventory screen seems to automatically move the double-clicked item from the inventory to the Q slot, I wonder if this is a case of the double-click triggering both behaviors—first it moves it to the Q wheel, and then from there to the storage?
I want to keep health potions int the Q slot. But every time i change realms or add a high tier meat or other high tier consumable it replaces the potions in this slot.
I built a new axe and put it in my hotbar. When I double clicked the old axe (in my inventory) to move it to storage, the old axe moved to storage and the new axe moved to my inventory. I was able to repeat this.
I was keeping health potions in the Q slot. Now it seems like when I pick up any other food, it gets swapped out. I've found myself in some dire circumstances because of this
As a note:
When testing this for someone I noted that since I right-click to fast move items, I had never run into the issue.
That being said, it's something to do with double clicking itself.

Saw that reported on PTR but hadn't got the bug until now. Happened in the boss rush after putting some loot in the pepys box. Was an hectic few seconds when I tried to chug a potion and found them empty XD
