Nightingale Community Portal
I teleported to The Watch for the first time, and my survivor wasn't with me. I didn't think anything of it since there were other players there until I noticed her calling card also wasn't in my inventory (I had not dismissed her since I did not know The Watch was not a realm she could go to prior to going.)
When I got back to my respite, she was outside to recruit again at least, but all her rare gear and great healing potions were gone. She was back in her starter gear, the rare nowhere to be found. I even went back to Gloriana's Tears hoping for a survivor chest, but alas there are none. I also checked The Crossways and the respite realm.
I'll make new gear for her, but was sad since I put a lot of time into making good stuff for her. I am unsure if this is repeatable.

I had a similar bug but with worse consequences
Steps for Repro:
1. Have a Recruit summoned wearing gear.
2. Teleport to the watch from Gloriana's tears portal
3. Enter a vault from the watch.
(This is where I noticed my recruit missing)
4. Summon another Recruit within the vault
5 (Optional) Mourne the loss of your recruit
Result.
My 1st recruit is nowhere to be found. Not at my respite. Not where I found the survivors chest with their belongings. Not at any realms Gloriana's tears Portal
Losing their well crafted gear is a huge hinderance to progression and because I had spent so long looking for my perfect partner npc its saddening to lose my "Friend"
