NPC recruit disappeared when i asked for her card, card did not go to inventory/card menu . Had gear on her. have had this recruit for months with no issues.
i also just tried to reproduce this bug and i did. second npc disappeared.
more info: i do have a t4 cairn at max capacity with workers if that helps.

Had a similar issue, dismissed my NPC recruit while trying to get an escort NPC to move (didn't realize I had to build them a staircase). In this case, my calling card slots were full but I didn't realize it and was not given any warning, causing my good friend Melvin to vanish into the pale forever.
Melvin was of course annoying at times, always in my face, taking his sweet time to pick things up while I stared intently wishing him to hurry up, but he had been with me for a long time and I had hoped to offer him a better retirement.
It's not a bug per-se as I now understand the card limit, but given that the NPC cards are in a separate tab entirely I wish it would have been communicated more clearly when I was dismissing him that I had to say my final goodbyes.
Also: my Respite estate cards were not at their limit when he was dismissed. To be fair, the estate was in Abeyance and I was in Nightingale so they were separate realms.
My uneducated solution for this problem: Would it be possible to have a separate "equipped NPC" card slot in the inventory UI allowing us to drag-and-drop the active recruit?

I have a couple of questions, if you don't mind:
Is your inventory for calling cards full? It should hold a maximum of 15 NPC calling cards. If you have more than that, any new cards will fail to be added to your inventory.
Do you have all of your NPCs assigned to jobs through the estate cairn?
