Created a masterwork hunting knife and mining pick. Added some enchantments, infusions, and charms, and upgraded. Looks nice. Until I cast Shockwave on the mining pick and I'm just pretty much blind indoors or outdoors.
Has also happened a couple of times in boss rush with the hunting knife, but usually only happens at the point of respawning from teleportation after the boss.
