Party of three; puck appeared to 2 out of 3 players. Quest logs are otherwise the same as well as challenges and codexes. Have tried the following:
log out, log back in
Travel to crossways
Log out at crossways
Try to progress the quest (works for the two coop members with the quest but other teammate cannot do anything).
Sleeping.
shutting down the server and restarting it.
