While helping someone else try to figure the bug, I also ran into this issue. It's specifically an issue with the tracking interface, as once you click untrack there's no going back, as far as I've seen.
Steps to reproduce: Have a character that has quests (As I was helping someone I made an alt) Untrack all quests with the available "untrack" button (which is only main story quests, sidequests are turned off by default, and thus can never be tracked.) Issue 1. The "track" button is greyed out. Issue 2. There's a confusing note as mentioned in this topic that there are to many quests tracked.

Yeah same for me cant add quests or remove that 1 quest like The Jurney home!

I'm also having this issue currently.
This gets super buggy with Co-Op and selected quests don't stay up for me and a friend when we were clearing storied realms content.
Perhaps, on default Main Questline gets priority to the top of the list.
