I've found that if you're climbing using picks and you're in a crevice, its possible that releasing the hold your picks has can transport you through walls and out of bounds where you fall to your death.

| Steps to Reproduce | In one cave (Gloriana's Tears X-16380, Y4817,7-7511) I found there is an area where its possible to climb up onto a ledge and if positioning yourself right you can wedge yourself with climbing picks. once you release yourself from climbing, you fall through the map. |
