This happened to me in an Ascended Swamp. A pillar spawned half way inside a wall and will always spawn bound even when activated in the correct order.
I have a similar issue. A Bastille of Intellect in an Astrolabe Forest realm has one spawned half inside a rock. It can be activated, but always spawns bound, even when activated in order.
