I've noticed that equipped tools and Q wheel items will unequip when dealing with inventories like chests. It took some prodding to figure out how to trigger it, but if you open an inventory like a chest and doubleclick a non-equipped tool to move...

The first slot of my consumables consistently unequips itself. I am not sure when it is happening, I notice it most often when I am unpacking inventory and opening and closing storage containers upon returning to respite.
