Completed Aid POI/Quest and reward box opens, but unable to claim essesence reward

When I talk to Wilhelmina Sasse or Graeme Turner for the quest "Friends in Unlikely Places", I only get a single introductory line of dialogue and no dialogue options. Similarly for the trader Lillian.

Her staff is there, hovering in the air. Her name-tag is there..but she herself is nowhere to be seen..perhaps it will sort itself out if i reset the world... -_-*

Can't find Puck after the Surviving the Faewilds quest. There's no quest marker on the map for it, so I have no idea where to go to find him. Edit: Rebooting the game caused him to show up for the missed interactions.

Players turn up at Welken's Reach, see The Well, jump into the centre. Every time I've done this I die inside but get ressed by my Follower inside The Well. So a new player will fight the boss, get the Magwytch card, get dropped at the Crossways.....

I opened a new card and then it says return to Puck, but I was off galivanting (didn't know where the card place was either until I came back). Why isn't Puck just appearing before me?

I got the achievement of throwing a snowball a long distance but it did not unlock the clothing? Did I miss something?
