unable to complete gear up quest, when right clicking the charm if the sniper, it says "no availble item to apply charm"

Portal back into completed personal vault resulted in spawning in mostly sky with some vault looking surrroundings but unable to enter. Using stuck or respawn point didn't resolve. Quit game and tried again with same result.
