Found a synchronous lotus chest in a player-opened swamp fae vault (balanced difficulty) that was pretty much inside the wall. Chest can still be opened, but only from a narrow angle. Not sure if debug screen is needed, so I've only attached the image of the chest for now.
Was found in a Bastille of Agility section of the fae vault, behind a cracked wall that we knocked down.
Friend and I did use a Dragon's Hoard Apogean minor realm card, though I doubt it caused this issue.
Have not encountered this a second time, but will provide more details in a comment if we do.
